The Mental Health Pharmacy Team comprises of pharmacists and technical staff and is based at Hafan y Coed at University Hospital Llandough.
Who we support
The Pharmacy Service provides both clinical and technical support to:
- Inpatient wards in Hafan y Coed, Llandough Hospital, Park Road and Phoenix House Rehabilitation units
- Adult Crisis Resolution and Home Treatment Teams and MHSOP REACT
- Inpatient Mental Health Services for Older People’s Wards, including Young Onset Dementia ward in Barry Hospital.
- Liaison Psychiatry for Older People (LPOP)
- Care Home Liaison Team (CHLT)
- Community Mental Health Teams
- Clozapine Clinics and Community Clozapine patients
- Perinatal Service
- Addictions Team as DATT
- Voluntary organisations such as HAFAL, Bipolar Wales and MIND
The services provided by the pharmacists include:
- Medicines reconciliation within 24 hours of admission
- Daily visits to the Crisis Assessment Ward and PICU ward to:
- Review prescriptions
- Check for drug interactions
- Check the appropriateness of medication
- Lead on the use of patient’s own medicines and facilitate self-administration on the wards
- Non-medical prescribing to patients in Community Mental Health Teams and within Mental Health Services for Older People / Young Onset Dementia, Care Home Liaison, LPOP, REACT, Perinatal and Addictions Service
- Provision of medicines information
- Attendance at Multidisciplinary Team meetings
- Acting as a second opinion
- Supporting discharge care planning
- Acting as an advocate for the patient
- Provision of information for the patient and/or carer
- Education and training for medical and other clinical staff
- Supporting Research and Audit within the Health Board and in conjunction with the Welsh School of Pharmacy
- Teaching to undergraduate and postgraduate pharmacy students in partnership with HEIW and Cardiff University
