Annual Review

You will be invited to attend for an annual review at some point during the year, hopefully around the same month as a year before.

Annual reviews have a nurse led clinic so no doctors are available on this day. You will roughly be in the clinic for 2 hours and in that time you will be seen by a specialist nurse, physiotherapist, dietitian, pharmacist, social worker/youth worker and psychology.

We are asking, if you are not a diabetic, to arrive in clinic nil by mouth from midnight. We will then do bloods straightaway, including an oral glucose tolerance test. You can drink water and take any medications.

During your time in clinic the members of the MDT will ask questions about your health and come up with a plan with you for the next year.

You may be asked to attend for a chest x-ray are and an abdominal ultrasound which we will try to book on the same day as your appointment. You may also receive an appointment for a Bone Density scan, which is at Medical Physics at the University Hospital of Wales, Cardiff.
We do ask if you are not  able to make the appointment to contact them to rearrange or cancel - Bone Densitometry (DXA Scan) - Cardiff and Vale University Health Board (nhs.wales)

Following your annual review, you will be invited back for a follow-up appointment where the consultant we will go through all your results and any queries you may have.

Annual reviews are held on a Wednesday every week, but if you are not able to make it her Wednesday please contact us to arrange an alternative day.

If you are not able to make your annual review appointment or the follow-up please can you contact us on 02920 715 946 to rearrange.
