If you wish to refer please fill out the Employee Wellbeing Self Referral Form.

If you would like to refer to the Employee Wellbeing Service, you can complete the Self-Referral Form by saving it to Your Documents, completing it and then returning it to employee.wellbeing@wales.nhs.uk, or by printing it out and sending by internal/external post if you prefer.

Once your form has been received, you will be contacted to arrange a resource appointment. This is an opportunity for you to come in to talk to a member of the team about the options available to you.
