CKD Service

We provide consultant led nephrology clinics throughout South East Wales. Our team of clinical nurse specialists support patients with more advanced kidney disease. We aim to inform and empower patients to take ownership of their own health and kidney condition. We support patients on their different kidney journeys and help patients to be involved in making decisions about what they want and what matters to them.

 

Living well with CKD

If you are a patient with Chronic Kidney Disease (CKD) we want to support you to understand what CKD means, how it may affect your life and what you can do to live well with CKD and possibly delay it getting worse. Medicines Optimisation Clinics

New medications have been developed recently which can help keep the kidneys and heart stay healthier for longer. Many, but not all, patients are on these medications already and we have special clinics to help ensure patients are aware of the link between CKD and a healthy heart. Our specialist pharmacy and nursing teams will educate patients, identify and offer help to modify risk factors such as smoking and high blood pressure, and coordinate the start of medications which are known to benefit the kidneys and heart.

 

ENHANCE - renal prehabilitation

Our Renal Prehab programme is aimed at identifying patients earlier who are at higher risk of worsening CKD. We help these patients identify areas of their health, wellbeing and life that we can improve and support to help them stay well, feel supported and empowered, and prepared should they reach the point where they need treatments for advanced CKD such as transplant or dialysis - more information is here.

 

Prepare - My Kidney Journey

If your kidney function is getting worse, there may come a time where we need to talk about plans for the future. A time when your own kidneys may not be able to keep you well and other treatments are needed such as transplant and dialysis.

The kidney journey options

See the links below for a brief summary of each treatment option for patients with advanced CKD - these same documents will be discussed with you by your doctor and specialist nurse in the kidney clinic. We want as many patients as possible to be able to have a kidney transplant. When dialysis is needed, we want that to be at home for as many patients as possible. Not all patients are suitable or want to consider all treatments and that will be discussed with you.
