We aim to provide the best possible care to all of our patients. However, should you wish to raise a concern or make a complaint about the care you have received in the CDS, you should do so verbally with one of our members of staff as soon as possible, and we will try our very best to rectify the problem. If we cannot resolve the issue and you wish to escalate your concern, please contact the Concerns Department and they will be happy to discuss your concerns with you.
The Concerns Office is open between the hours of 9am to 5pm (Monday to Friday). Please call on the following telephone numbers in office hours if you wish to speak with a member of the Concerns Team.
You can also fill in our Concerns Form, e-mail the team at firstname.lastname@example.org or write to us at Chief Executive, Cardiff and Vale University Health Board, Maes y Coed Road, Llanishen, Cardiff CF14 4HH.
Equally, we like to know if we are doing things well. If you are really happy with the treatment you have received from a particular member of staff, please let us know!
You may submit a compliment in writing to:
Cardiff and Vale University Local Health Board Headquarters
University Hospital of Wales (UHW)
or by email to email@example.com
You will receive a letter or e mail in response to your compliment, and all compliments are shared with the areas mentioned and individual staff members.
