Stop a Stroke - How?

A project team led by a consultant haematologist, consultant stroke physician and anticoagulation specialist pharmacist has worked to create a partnership with Primary Care. 

Their vision is to enable patients with atrial fibrillation to avoid having a potentially debilitating stroke by supporting Primary Care practitioners to identify and review their anticoagulation status.

  1.  The Stop a Stroke team will visit your cluster meeting to initiate the review process.  This will include showing the data available on the number of patients with atrial fibrillation potentially not treated in line with NICE guidance in your area and outlining the support available from Secondary Care.
  2.  Following this you can access the AF AuditPlus module (see Resources) to find the patients in your practice who may require an anticoagulation review.
  3. Work through this list to identify those who need an appointment for discussion of treatment decisions.
  4. Use the online resources, frequently asked questions documents and e-advice to help you through this process (see Resources).
  5. A virtual clinic for complex cases will be offered by the Stop a Stroke team at a time convenient for your cluster.
  6. Use the resources on an ongoing basis to assist with the anticoagulation of new atrial fibrillation patients.
