We accept self-referrals and referrals from all health professionals (with the person's consent) for diagnostic assessments and for support if someone has a diagnosis.
Please complete the referral form and send it to us.
For those who already have a diagnosis we accept self-referrals, referrals from parents and carers and referrals from professionals. We will ask that you provide evidence of your diagnosis, ideally a diagnostic report. Referrals can be made by email on cav.ias@wales.nhs.uk, by phone on 07970 647 820, or in writing. Please email us if you would like us to send you a referral form for support.
Diagnostic assessments: When we have received your referral, we will process the referral and then you will be written to with information about the process and asked to opt-in. You need to inform us that you wish to proceed. From September 2022, all new referrals for assessment are invited to a triage appointment.
Please note, the triage clinic will offer rapid access to the service; however, should individuals need further assessment the waiting time is still likely to be around 24 months at the present time.
Other referrals: If appropriate, you will be offered an initial appointment with a member of our team who will discuss what support . The outcome of that appointment may be that we offer you group support, signpost you to other services, or offer you some individual support.
