District Nursing Service

The District Nursing Service is part of Cardiff and Vale University Health Board. District Nurses are employed by the UHB and work closely with GPs and other health and social care professionals within the local community. They also have a close working relationship with the voluntary sector.

The aim of the service is to provide skilled nursing care to patients in their own homes, by specially trained community nurses, and to provide support to carers. The service is provided over the 24 hour period, 365 days a year, and is available currently to all individuals over the age of 16 years of age.

The District Nursing service also has specialist links to all community based teams including the Acute Response Team, which provides an early hospital discharge scheme or prevention of admission service. Specialist Wound Care and Continence Advisory Services are also available.

District Nurses are based throughout the community of Cardiff and Vale, mostly in Community Clinics and a few in GP premises.

Important Notice regarding changes to the District Nursing contact centre (Communication Hub)

We have recently been conducting an audit of the calls coming in to the Communication Hub which has identified that approximately 30% of calls (around 30,000 calls per annum) ask if and at what time the District Nurse is calling. Whilst the District Nurse will endeavour to call on the day requested, urgent calls need to take priority, eg for palliative patients, and therefore we cannot generally specify a time when the District Nurse will call.

As a result of the above the Communication Hub answer message will read as below:

"Welcome to Cardiff and Vale District Nurse booking service. Please note that the District Nurse service is for housebound patients only and daytime nurse visits can take place anytime from 8am to 8pm, Monday to Friday. Outside of these hours the District Nurse service will deal with emergency calls only. If you are calling to ask what time or if the District Nurse is calling, your GP surgery or the Contact Centre are not able to supply you with this information.

Please ensure you have all your or the patients personal details and the reason for your call. Your call is being transferred to an operator who will assist you with your enquiry, please note all calls are recorded for your security."

Please note the key messages below:

  • District nursing visits are for housebound patients only; housebound does not include those without transport.
  • Times of calls cannot be given (unless in specific circumstances or because of time critical interventions and these are agreed in advance).
  • The communication hub is unable to give day/ time of District Nursing visits, as are GP practices.
  • DN day staff work Monday to Friday, 8am to 8pm.
