Registering with a GMS Practice

GP Card

How to register with a GMS Practice

Anyone in Wales can register with a GP for treatment (see link below for GP's available in Cardiff and Vale).

The only form that needs to be completed in order to register is a GMS1 form (see useful links below) which are available at any practice.

It is not necessary to have a fixed address or identification to register and anyone who is registered with a GP can get free prescriptions.

Anyone who needs an interpreter has the right to request one at no cost to themselves. The GP practice has a duty to provide a translator and telephone translation services are available in all GP practices.

Unless the GP practice has closed their list or you live outside the practice boundary area, you cannot be refused registration.

If a surgery wants to refuse to register you, it must have good reasons for doing so. These must not have anything to do with race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

If you think you have been refused registration for any of the following reasons; race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition then this may be discrimination.  For more information regarding discrimination in health and care services please visit citizens advice.
