The In-Reach Team helps people from health inclusion groups who go to the Emergency Unit (EU) or stay in the University Hospital of Wales (UHW).
This includes people:
- Who have moved to the UK from other countries seeking refuge or asylum
- Who are homeless
- Who have just come out of prison and meet certain eligibility criteria.
- Gypsy, Roma and Traveller communities on unauthorised encampment sites.
- People affected by sex work.
❤️ What does the team do?
The team works to:
- Make it easier for people to get the care they need
- Help people leave hospital safely
- Reduce long hospital stays
- Support people who find it hard to use regular health services
They work with:
- Hospital staff
- Housing teams
- Charities and support organisations
🕒When and Where?
- The team is based at University Hospital of Wales (UHW)
- They work Monday to Friday, from 8:00 AM to 4:00 PM
👩⚕️ Who is in the team?
The team includes:
- Nurses who:
- Give medical support
- Help people get the right treatment
- Work with hospital and community teams to plan safe discharge
- A GP (doctor) who:
- Visits the hospital.
- Gives health checks and care
- Leads meetings to make sure people get full support from different teams.
- Helps with discharge planning
