In-Reach Services

The In-Reach Team helps people from health inclusion groups who go to the Emergency Unit (EU) or stay in the University Hospital of Wales (UHW).

This includes people:

  • Who have moved to the UK from other countries seeking refuge or asylum
  • Who are homeless
  • Who have just come out of prison and meet certain eligibility criteria.
  • Gypsy, Roma and Traveller communities on unauthorised encampment sites.
  • People affected by sex work.
 

❤️ What does the team do?

The team works to:

  • Make it easier for people to get the care they need
  • Help people leave hospital safely
  • Reduce long hospital stays
  • Support people who find it hard to use regular health services

They work with:

  • Hospital staff
  • Housing teams
  • Charities and support organisations
 

🕒When and Where?

  • The team is based at University Hospital of Wales (UHW)
  • They work Monday to Friday, from 8:00 AM to 4:00 PM
 

👩‍⚕️  Who is in the team?

The team includes:

  • Nurses who:
    • Give medical support
    • Help people get the right treatment
    • Work with hospital and community teams to plan safe discharge
  • GP (doctor) who:
    • Visits the hospital.
    • Gives health checks and care
    • Leads meetings to make sure people get full support from different teams.
    • Helps with discharge planning
