The Mental Health Pharmacy Team, comprising of pharmacists and technical staff, is based at Hafan y Coed, Llandough Hospital, and can be contacted on 029 2182 4804.
Who we support
The Pharmacy Service provides support, both clinical and technical, to:
- the wards in Hafan y Coed, Llandough Hospital
- the Crisis Resolution and Home Treatment Teams
- the Mental Health Services for Older People’s Wards
- the Community Mental Health Teams
- the Clozapine Clinics and Community Clozapine patients
- voluntary organisations such as HAFAL, Bipolar Wales, MIND.
The services we provide
The services provided by the pharmacists include:
- Medicines reconciliation within 24 hours of admission
- Daily visits to the Crisis Assessment Ward and Locality Wards to:
- review prescriptions
- check for drug interactions
- check the appropriateness of medication
- Lead on the use of people’s own medicines and facilitate self administration on the wards
- Non medical prescribing to clients in Community Mental Health Teams and within Mental Health Services for Older People / Young Onset Dementia
- Provision of medicines information
- Attendance at Multidisciplinary Team meetings
- Acting as a second opinion
- Supporting discharge care planning
- Acting as an advocate for the client
- Provision of information for client and / or carer
- Education of medical and nursing staff
- Supporting Research and Audit within the Health Board and in conjunction with the Welsh School of Pharmacy
