You cannot register a death or obtain a Death Certificate at the hospital. For deaths at University Hospital of Wales; the death can be registered at:

If you live in a different area it is possible to register a death with a Registrar local to you, contact the Registrars for more information.
It is a legal requirement to register the death within five working days of the Medical Certificate of Cause of Death being issued from the hospital. This period will be extended if the death is referred to the Coroner’s Service.
Who can register a death:
-someone who was present at the death
-a representative of the establishment/hospital in which the death occurred
-the person instructing the funeral director.
The Registrar will issue a certificate (known as the green form). This will be required by the funeral director to give them the authority to continue with the arrangements with the funeral. You will be issued a Death Certificate, there is a fee for copies of this. You may require copies for banks, building societies, insurance companies, solicitors or for pension claims etc.
