Information on outpatient referrals

Please use the Welsh Clinical Communications Gateway (WCCG) national referral template whenever possible to send electronic referrals. 

Where possible, send all outpatients referrals via the electronic gateway (WCCG) and choose Inherited Metabolic Medicine Service and University Hospital of Wales.

Tertiary referrals

When not using the WCCG national referral template please ensure that all other referrals are sent via email to Service.Coordinators.Cav@wales.nhs.uk

Please ensure that the following information are included in all referrals:

• Full patient details with contact telephone numbers
• Nature and duration of the condition
• Relevant past medical history
• Any previous height and/or weight measurements with dates
• Results of any relevant investigations
• Medication history
