Advanced Nurse Practitioner

Siji Salumkutty began his nursing career in 1994 after completing training in Kerala, India. He now works as an Advanced Nurse Practitioner in Primary Care at Cardiff and Vale University Health Board.

After moving to Wales, Siji pursued a Master's Degree at the University of South Wales, qualifying as an advanced Nurse Practitioner and Independent Prescriber.

“Over the years, I have embraced various opportunities, from clinical leadership to mentoring and workforce advisory roles” says Siji.

“My journey has been guided by a commitment to continuous learning and patient-centred care, leading to my current role, where I support the elderly community, reduce hospital admissions, and contribute to innovative healthcare delivery.”

As an Advanced Nurse Practitioner in Primary Care, Siji works autonomously, managing complex cases, providing advanced diagnostics, patient care, and independent prescribing.

“My role is uniquely integrated with the Safe@Home team, focusing on supporting the elderly community by delivering tailored interventions to prevent unnecessary hospital admissions and promoting their well-being within their own homes.”

“Collaborating with multidisciplinary teams, particularly the Safe@Home team, allows me to create meaningful impact by enabling elderly patients to receive the care they need at home” he says.

“Helping to maintain their independence while reducing hospital admissions is incredibly rewarding.”

“I chose this path because uplifting others brings me joy, and through nursing, I can make a meaningful difference every day. Working with the elderly community to help them stay safe at home reinforces my belief that compassion and care can transform lives and healthcare systems alike.”
