About the Primary Care Prehab2Rehab Health Optimisation Service


The Health Optimisation Service is designed to enhance the health and wellbeing of Cardiff and Vale residents who have been referred to the hospital for investigations and potential treatment. Currently, this service is exclusively available in the Cardiff South West Cluster.


Our Approach

This virtual service provides patients with a comprehensive health optimisation review conducted by a therapy assistant practitioner and a medication review by a clinical pharmacist. Our goal is to improve overall health outcomes by focusing on key areas such as:

  • Smoking cessation
  • Exercise and physical activity
  • Alcohol consumption
  • Weight management
  • Mental wellbeing

Patients receive tailored advice and support to foster positive behavioural changes, leading to better long-term health. Additionally, the service offers signposting to local services and resources, ensuring patients have access to the support they need.
