Palliative Care is available to patients whose disease or illness is no longer responsive to curative treatment. This may include those affected by cancer or a non-cancer illness, for example end stage lung disease, heart disease, motor neurone disease and multiple sclerosis.
The hospital palliative care team is based at the University Hospitals of Wales and University Hospital Llandough and covers Barry Hospital, St. David’s Hospital and Cardiff Royal Infirmary to provide a service to patients and their families.
Through an integrated, multi-professional service, the team aims to respond effectively, offering specialist advice and support within the hospital. Through collaborative working they will support, educate, encourage and empower other staff members to provide effective care. The team also provides links with the Community Palliative Care teams and local hospices, ensuring seamless care and support between the hospital and the community.

Community Specialist Palliative Care is provided locally by two different providers who deliver similar services to patients and their families in their own homes, in care homes or in a specialist palliative care centre such as the Marie Curie Hospice.
The Hospital Palliative Care Team will facilitate referral to one of these organisations (below) when patients are due for discharge from hospital. This will be carried out in consultation with the patient's GP and District Nurse.

We know how important compassionate, respectful and dignified end-of-life care is for patients and their loved ones. To support this, the Hospital Palliative Care Team has introduced ‘Every Moment Matters’ to help create a calmer, more peaceful environment during this significant time.
‘Every Moment Matters’ is an initiative designed to enhance dignity and respect for patients in the last days of life. As part of this, a new symbol will be introduced across our clinical areas to raise awareness when a patient is receiving end-of-life care.
If a patient or their loved ones choose to display this symbol, it will serve as a gentle reminder for other patients, visitors and colleagues to be mindful and to approach the area with additional awareness, sensitivity and respect. If you see this symbol displayed in a patient’s area, please be mindful of noise levels, and consider those nearby.
