How to Nominate

Nominating your Health Hero is quick and easy.

Complete the Health Hero Nomination Form, providing as much information as possible.

The more detail you provide, the better we can understand their contribution and the impact they’ve had.

Health Hero is proudly sponsored by Park Plaza Cardiff. Each winner will receive a free experience for themselves and a guest, giving them a well-deserved moment to relax and unwind.

We will announce a new Health Hero every month. Please keep an eye on our social media channels and website for future announcements. If you submit a nomination and they win, we will also contact you directly.

Unfortunately, we can only consider nominations for individuals and cannot accept nominations for groups or team.
