Primary Care Liaison Service

Since May 2019 the Adult Mental Health Board, in partnership with Primary Care and GP practices of Cardiff & Vale UHB have been placing Mental Health Specialist Practitioners into GP surgeries to assist GPs and their patients who present with mental health problems. These highly experienced staff work within GP practices to see patients at the GPs request to assist them in understanding what the nature of the problem is and also what the best and most efficient route to recovery is for the patient.


In many cases the issues can be resolved within the practice without the need of a referral to another service or agency. Our staff are very familiar with all the other agencies and services available within the community, including Social Prescribing agencies in the Charity sector, as well as the various clinical pathways through the Health Board.


The Health Board options include web-based services, advice, guidance and support, as well as more structured assistance from a range of community-based services that offer Cognitive behavioural Therapy, Counselling, Guided Self-help, bereavement support, anger management as well as access to and information about Groups and services aimed at addressing housing, debt, employment, abuse recovery, domestic violence support.


Our Mental Health Practitioners will also be able to assist with referrals to other services such as Community Mental Health Teams, Psychological Therapies, Primary Mental Health Support Services and specialist Trauma services.


 Mental Health Practitioners are available in every GP practice in Cardiff and the Vale. Ask at your GP practice if you think we might be able to help, no referral, no waiting and within your own local and familiar GP surgery. Our aim is to assist people in making sense of what might be troubling them and finding a positive option to bring about change. The initial contact will be by telephone consultation with face-to-face consultations following if necessary. This means that addressing your mental health concern can be as convenient and discrete as a simple telephone call.
